UnitedHealthcare Global App

Use myuhc.com and/or the UnitedHealthcare Global app to:

• Find the average cost of care
• See what’s covered
• Submit and view claim details
• Check your plan balances
• Find network providers

ID cards

• An ID card will be sent to you in the mail.
• Always carry your ID card with you.
• You can also find your digital ID card on myuhc.com and in the UHC Global app.
• Once you receive your card, activate your account by following the step-by-step instructions at myuhc.com.

Canada

• UHC Global will send a Welcome Kit and ID cards to eligible employees on the Kiewit International Plan who are being transferred to Canada.
• UHC Global has partnered with Cowan, an insurance company that has its own network of providers, in-network and out-of-network. You will receive a separate welcome email with an ID card from Cowan.
• Show the card from Cowan to your providers while in Canada. When in U.S. show the UHC Global card.

Puerto Rico

• MAPFRE is the network that allows employees working in Puerto Rico to receive services from MAPFRE network providers.

When you enroll in the Kiewit International Plan, dental coverage is included. You can use the services of any dentist or dental specialist and do not need a referral. The provider is UnitedHealthcare Global Solutions and coverage includes:

• 100% for preventive services (two times per consecutive 12 months)
• 80% for basic covered services
• 50% for major covered services
• 50% for orthodontia care (up to $1,500 lifetime for covered children and adults)

There is a $25 deductible ($75 for family) that applies to covered basic and major services only. The annual benefit maximum of $1,500 also applies only to covered basic and major services.

When you enroll in the Kiewit International Plan, prescription drug coverage is included through UnitedHealthcare Global Solutions. The plan offers:

• Flexibility to receive necessary medications both in and outside of the U.S.
• Added convenience of receiving up to a one-year supply of maintenance medications prior to leaving the U.S.

UnitedHealthcare Virtual Visits connects you to doctors and specialists through secure video, chat or email for a wide range of treatments and therapies.

• Find support when you need it, in a way that works best for you.
• Request a visit and get care sooner, as early as same day for some providers.
• Virtual visits are covered like any other medical expense.
• Sign in to myuhc.com or use the UHC Global app to find and connect with a doctor.

Getting a comprehensive eye exam each calendar year does more than test your vision. It can also identify symptoms of many health problems, including diabetes, hypertension, high cholesterol, glaucoma and cataracts. The UHC Global Solutions Plan includes:

• 100% coverage for an eye exam each calendar year in the U.S. (up to $80 outside of the U.S.).
• Up to $110 for frames and up to $150 for contacts outside of the U.S.
• Up to $130 for frames and full coverage for contacts in the U.S. network.
